Appointment needed. All arrangements should be made at least 6 months in advance to allow time for interviews, instruction and preparation. |

| Anointing of the Sick |

We are anxious to serve anyone who cannot celebrate mass with us in church due to illness, handicap or age. If you wish to receive any Sacrament (Communion, Confession, Anointing of the Sick) please contact the church office. |

| Sponsor Certificates/Affirmation |
|
In order to receive a Sponsor Affirmation form for Baptism or Confirmation, you must be an active parishioner registered with St. Ambrose Parish for a minimum of 6 months. |
